secondly, I see that you've applied the SQL scripts up to version 4.11, but it looks like you may have applied one of the Flyway scripts manually. Flyway maintains an internal table of what database updates need to be applied, so you may want to try again without that Flyway script, to let Flyway do its thing

you need to run each update until Flyway comes on the scene

I personally would only step through each update, even after Flyway, but in theory you can then jump to the latest release, you just have to check all the additional steps in each release in between
